A fantastic opportunity has arisen for an experienced Adults Social Worker to join an Integrated Community Care Team in Birkenhead on a long-term maternity cover contract.
Working within a supportive multi-disciplinary team, you will play a key role in delivering health and social care services to adults and carers, ensuring the principles of the Care Act 2014, strengths-based practice, personalisation and social inclusion are embedded in all areas of practice.
Key DutiesCompleting Care Act AssessmentsUndertaking support planning and reviewsManaging adult safeguarding workSupporting adults with complex physical health, mental health and long-term care needsProviding advice, information and guidance to adults and carersSupporting and mentoring ASYE Social Workers and Care NavigatorsRequirementsSocial Work England registrationExperience within Adults ServicesStrong knowledge of the Care Act 2014Experience completing Care Act assessments, reviews, support plans and safeguarding enquiriesWhy Apply?
