Join our dedicated team at the Birkenhead Integrated Community Care Team (ICCT) in Wirral as a Qualified Social Worker. We are seeking a compassionate and motivated individual to support and empower our diverse community. As a Social Worker, you will play a crucial role in assessing needs, developing care plans, and delivering high-quality services to individuals and families facing various challenges.

Your key responsibilities will include:

Conducting comprehensive assessments of service users' needs and circumstances. Developing, implementing, and reviewing care plans in collaboration with clients and other professionals. Providing support and guidance to individuals and families, ensuring their voices are heard and respected. Working closely with multidisciplinary teams to ensure integrated service delivery. Maintaining accurate and up-to-date records in compliance with relevant legislation and policies. Participating in training and professional development opportunities to enhance your practice.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in a social work role, preferably within community settings. Strong communication and interpersonal skills, with the ability to build rapport with clients from diverse backgrounds. Excellent organizational skills and the ability to manage a varied caseload effectively.

Working within a supportive multi-disciplinary team, you will play a key role in delivering health and social care services to adults and carers, ensuring the principles of the Care Act 2014, strengths-based practice, personalisation and social inclusion are embedded in all areas of practice.
