Your key responsibilities will include:
Conducting comprehensive assessments of individuals' needs and circumstances. Developing and implementing person-centered care plans in partnership with clients and their families. Providing ongoing support and guidance to clients, ensuring their voices are heard in decision-making processes. Collaborating with multidisciplinary teams and external agencies to coordinate care and services. Maintaining accurate and up-to-date case records and documentation in compliance with legal and organizational standards.To be successful in this role, you will need the following essential qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working with adults in a community setting, demonstrating knowledge of relevant legislation and best practices. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
