Join our dedicated team as a Social Worker in the Adults Community sector in Altrincham, Trafford, where you will play a vital role in improving the lives of adults in our community. You will work collaboratively with individuals and their families, assessing needs and providing tailored support. Your expertise will be instrumental in promoting independence and well-being for vulnerable adults, ensuring they receive the appropriate services and resources to thrive.

Your key responsibilities will include:

Conducting comprehensive assessments of individuals' needs and circumstances. Developing and implementing person-centered care plans in partnership with clients and their families. Providing ongoing support and guidance to clients, ensuring their voices are heard in decision-making processes. Collaborating with multidisciplinary teams and external agencies to coordinate care and services. Maintaining accurate and up-to-date case records and documentation in compliance with legal and organizational standards.

To be successful in this role, you will need the following essential qualifications and skills:

A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working with adults in a community setting, demonstrating knowledge of relevant legislation and best practices. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders. Excellent organizational and time-management skills, with the capacity to manage a diverse caseload effectively.
