Your key responsibilities will include:
Conducting comprehensive assessments of adults’ needs and risks to inform effective care planning. Developing, implementing, and reviewing care plans in partnership with service users and their families. Providing information, advice, and support to individuals to empower them in making informed decisions. Maintaining accurate case records and documentation in accordance with regulatory standards. Collaborating with multidisciplinary teams, external agencies, and community resources to ensure holistic service delivery.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work or equivalent). Registration with Social Work England. Demonstrated experience in adult social work, with a strong understanding of relevant legislation and best practices. Excellent communication, interpersonal, and problem-solving skills.
