Join our dedicated Children's TM - Care Leavers Team in Southwark, where you will play a pivotal role in supporting young individuals transitioning from care to independent living. As a Qualified Social Worker, you will provide tailored support and guidance to care leavers, ensuring they have the resources and skills necessary to thrive in their new environments. Your expertise will contribute to positive outcomes for vulnerable young people, helping them navigate the challenges of adulthood.
Your key responsibilities will include:
Conducting assessments and developing personalized support plans for care leavers. Building trusting relationships with young people, fostering their confidence and independence. Collaborating with multi-agency teams to ensure a holistic approach to care and support. Monitoring progress and adapting plans as necessary to meet individual needs. Providing advice and advocacy to empower young people in accessing education, employment, and housing.To be successful in this role, candidates must possess the following qualifications and skills:
A relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience working with children and young people, particularly care leavers. Strong communication and interpersonal skills, with the ability to engage effectively with diverse groups.
