As a Children’s Social Worker in the Transitions Team, your key responsibilities will include:
Assessing the needs of young people and their families, developing tailored support plans. Collaborating with multi-agency teams to ensure seamless transitions and continuity of care. Providing emotional and practical support to young people, empowering them to make informed choices. Monitoring and reviewing progress, adapting support plans as necessary to meet changing needs. Maintaining accurate and up-to-date records in compliance with statutory requirements and best practices.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., CQSW, DipSW, BA Hons in Social Work). Current registration with Social Work England. Experience working with children and young people, particularly in transition planning. Strong communication skills, with the ability to engage effectively with diverse stakeholders.
