Are you a dedicated and compassionate social worker looking to make a meaningful impact in the lives of children and families? Worcestershire County Council is seeking a qualified Children’s Social Worker to join our Transitions Team. This role is pivotal in supporting young people as they navigate the transition from childhood to adulthood, ensuring that they receive the guidance and resources they need to thrive.

As a Children’s Social Worker in the Transitions Team, your key responsibilities will include:

Assessing the needs of young people and their families, developing tailored support plans. Collaborating with multi-agency teams to ensure seamless transitions and continuity of care. Providing emotional and practical support to young people, empowering them to make informed choices. Monitoring and reviewing progress, adapting support plans as necessary to meet changing needs. Maintaining accurate and up-to-date records in compliance with statutory requirements and best practices.

To be considered for this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., CQSW, DipSW, BA Hons in Social Work). Current registration with Social Work England. Experience working with children and young people, particularly in transition planning. Strong communication skills, with the ability to engage effectively with diverse stakeholders. Sound knowledge of relevant legislation and safeguarding practices.

We are seeking a dedicated and compassionate Children's Social Worker to join our Transitions Team in Worcestershire. This pivotal role involves supporting young people with complex needs as they navigate critical transitions in their lives. You will work collaboratively with families, schools, and other agencies to ensure that the best outcomes are achieved for each child, helping them to thrive during these crucial stages of development.

Key responsibilities include:

Conducting comprehensive assessments to identify the needs of children and young people. Developing and implementing tailored support plans that promote positive outcomes. Building strong relationships with children, families, and professionals to foster a supportive network. Monitoring and reviewing progress, making adjustments to support plans as necessary.
