Your key responsibilities will include:
Conducting assessments and developing tailored support plans for young individuals leaving care. Providing guidance and practical assistance in areas such as housing, education, and employment. Building trusting relationships with young people to empower them in making informed decisions. Working closely with other professionals, families, and community resources to ensure holistic support. Maintaining accurate records and documentation in compliance with local policies and procedures.The ideal candidate will possess essential qualifications and skills, including:
A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Care Wales. Experience in working with children and young people, particularly in a leaving care context. Strong communication and interpersonal skills, with the ability to engage effectively with diverse individuals.
