We are seeking a qualified Children’s Social Worker to join a dedicated Leaving Care Team supporting young people as they transition from care into adulthood. This is an opportunity to make a meaningful difference by helping care-experienced young people develop the confidence, skills and stability needed to achieve their goals and live independently. You will work with young people aged 16 and above, including those eligible for leaving care support, care leavers and young people with complex or additional needs.
Your responsibilities will include assessing needs, preparing and regularly reviewing pathway plans, and providing consistent, relationship-based support tailored to each young person. You will help young people access suitable accommodation, education, employment, training, health services and financial support, while promoting their safety, wellbeing and participation in decisions about their lives. You will maintain accurate case records, complete statutory documentation, contribute to safeguarding planning and work collaboratively with families, carers, housing providers, education services, health professionals and other partner agencies. You will also advocate for young people and ensure that their views, wishes and identity remain central to all planning.
Applicants must hold a recognised Social Work qualification and current professional registration, with relevant post-qualifying experience in children’s social care, leaving care or a related setting. You should have a sound understanding of legislation, safeguarding procedures, corporate parenting responsibilities and the challenges faced by care-experienced young people. Strong assessment, communication, organisation and report-writing skills are essential, together with the ability to manage a varied caseload, work autonomously and make balanced professional decisions. We are looking for a resilient, compassionate and proactive practitioner who can build trusting relationships, promote independence and contribute positively to a supportive multidisciplinary team. A valid driving licence and willingness to travel between community locations may be required.
