Your key responsibilities will include:
Conducting comprehensive reviews of existing care plans and service provision. Assessing the needs and preferences of adults with varying levels of support requirements. Developing and implementing person-centered care strategies. Maintaining accurate and up-to-date records in compliance with local and national standards. Participating in multi-disciplinary team meetings and contributing to service improvement initiatives.To be considered for this role, you must possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work or equivalent). Registration with Social Work England. Proven experience in adult social care, particularly in assessment and review processes. Strong communication and interpersonal skills, with the ability to build rapport with service users.
