We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Reablement Team in Leicester. This pivotal role involves working closely with adults to promote independence and enhance their quality of life through tailored support plans. You will be a vital part of a multidisciplinary team, committed to delivering high-quality services that empower individuals to regain their autonomy.

Key responsibilities include:

Conducting comprehensive assessments to identify the needs and goals of service users. Developing and implementing personalized reablement plans in collaboration with clients and their families. Providing ongoing support and guidance, ensuring that clients are engaged in their care process. Monitoring and reviewing progress, adjusting plans as necessary to achieve optimal outcomes. Working collaboratively with other professionals and agencies to ensure a holistic approach to care.

The ideal candidate will possess the following essential qualifications and skills:

A recognized Social Work qualification (Degree in Social Work or equivalent). Registration with Social Work England. Proven experience in adult social work, particularly in reablement or rehabilitation settings. Strong assessment and analytical skills, with the ability to think critically. Excellent communication and interpersonal skills, fostering positive relationships with clients and colleagues.
