As an Adult's Social Worker, your key responsibilities will include:
Conducting comprehensive assessments to identify the needs and aspirations of young adults with disabilities. Developing and implementing support plans tailored to individual needs, ensuring a person-centered approach. Coordinating with various stakeholders, including families, healthcare providers, and community organizations, to facilitate the transition process. Providing ongoing support and guidance to young adults and their families throughout the transition to adulthood. Maintaining accurate case records and documentation in compliance with local and national standards.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent). Current registration with Social Work England. Experience in working with young adults with disabilities or in a similar field. Strong communication and interpersonal skills, with the ability to build rapport with clients and their families.
