We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work - Physical Disabilities (PD) Team in Leicestershire. This is an exciting opportunity for an experienced professional to make a meaningful impact in the lives of adults with physical disabilities. You will be working within a supportive team, committed to promoting independence and enhancing the quality of life for our service users.
Key responsibilities will include conducting assessments, developing and implementing care plans, and providing ongoing support to individuals and their families. You will collaborate with various agencies and stakeholders to ensure a holistic approach to care, advocating for the best interests of your clients. Additionally, you will be responsible for maintaining accurate records and documentation in accordance with local policies and procedures.
Conduct comprehensive assessments of service users' needs. Develop, implement, and review tailored care plans. Provide emotional and practical support to individuals and their families. Collaborate with healthcare professionals and community services. Maintain accurate case documentation and reporting.To be considered for this position, you must have a recognized social work qualification (e.g., BA/MA in Social Work) and be registered with Social Work England. A minimum of two years' experience in adult social work, particularly with physical disabilities, is essential. You should possess excellent communication and interpersonal skills, a strong understanding of safeguarding practices, and the ability to work effectively within a multidisciplinary team.
