We are seeking a qualified Adult Social Worker to join a neighbourhood-based team supporting adults to live as independently, safely and meaningfully as possible within their communities. You will manage a varied caseload, completing strengths-based assessments, reviewing support arrangements and identifying eligible needs in line with relevant legislation, statutory guidance and local procedures. The role will involve working with adults, families, carers and partner agencies to develop personalised plans that promote choice, wellbeing, independence and positive outcomes.
Your responsibilities will include undertaking proportionate assessments, safeguarding enquiries and mental capacity assessments where required, while applying sound professional judgement to manage risk. You will coordinate multi-agency meetings, arrange and review commissioned services, maintain accurate and timely records, and ensure that decisions are clearly evidenced. You will also contribute to hospital discharge planning, prevention and early intervention work, and support people to access community resources, informal networks and appropriate specialist services. Effective communication and collaborative working will be essential when liaising with health professionals, housing providers, voluntary organisations and other services.
Applicants must hold a recognised Social Work qualification and current registration with the relevant professional regulator. You should have experience of adult social care practice, including assessment, care planning, safeguarding, strengths-based approaches and relevant legal frameworks. We are looking for someone who can prioritise competing demands, work confidently both independently and as part of a team, and respond sensitively to complex or changing circumstances. Strong written and verbal communication skills, sound analytical ability and a commitment to equality, dignity and person-centred practice are essential. A valid driving licence and the ability to travel across the local area may be required. This is an opportunity to make a direct difference to adults and carers while contributing to a supportive, reflective and improvement-focused service.
