We are currently seeking a dedicated and experienced Adult's Social Worker to join our Mental Health Team in Liverpool. This position offers the opportunity to make a real difference in the lives of vulnerable adults in the community.

Key responsibilities and duties:

Conduct comprehensive assessments of individuals' mental health needs and develop personalized care plans. Provide ongoing support and intervention to promote mental well-being and independence. Collaborate with multi-disciplinary teams, including healthcare professionals and community partners, to ensure holistic care for service users. Advocate for clients' rights and empower them to access appropriate services and resources. Maintain accurate and up-to-date records in compliance with legal and regulatory standards.

Essential qualifications and skills:

Qualified Social Worker with a valid registration with Social Work England. Demonstrated experience working with adults with mental health issues in a social work setting. Strong assessment and care planning skills, with a person-centered approach. Excellent communication and interpersonal skills to build rapport with clients and stakeholders. Ability to work independently and as part of a team in a fast-paced environment.
