We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Locality Team in Wokingham. This is an exciting opportunity for a professional who is committed to improving the lives of adults within our community. You will play a vital role in assessing needs, developing care plans, and providing ongoing support to individuals and families facing various challenges.

Key responsibilities include:

Conducting comprehensive assessments to identify individual needs and risks. Developing and implementing person-centered care plans in collaboration with service users and their families. Providing ongoing support and monitoring of care plans to ensure effective service delivery. Working closely with multidisciplinary teams and external agencies to promote the welfare of adults. Keeping accurate and up-to-date case records in accordance with local policies and procedures.

To be considered for this role, you must possess the following qualifications and skills:

A relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working in adult social care, with a strong understanding of safeguarding and mental capacity legislation. Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals. Strong organizational skills and the ability to manage a caseload effectively.
