Are you a dedicated and passionate social worker looking to make a meaningful impact in the lives of adults in Somerset? Join our Locality Team as a Qualified Social Worker and contribute to the well-being and empowerment of individuals in your community. In this role, you will work collaboratively with service users, families, and other professionals to assess needs, develop care plans, and ensure the delivery of high-quality support services.
Your key responsibilities will include:
Conducting comprehensive assessments of adult service users to identify their needs and strengths. Developing and implementing tailored care plans in partnership with service users and their families. Providing ongoing support and advocacy, ensuring that individuals receive the appropriate services and resources. Collaborating with multidisciplinary teams, including health services, housing, and community organizations. Maintaining accurate records and documentation in accordance with local policies and regulations.To be successful in this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work) and current registration with Social Work England. Proven experience in adult social work, particularly within a locality team setting. Strong assessment and analytical skills, with the ability to develop effective care plans. Excellent communication and interpersonal skills, with a commitment to person-centered practice.
