In this role, you will work collaboratively with individuals, families, and multidisciplinary teams to assess needs, develop care plans, and provide support to those facing complex challenges.
Your key responsibilities will include:
Conducting comprehensive assessments to identify the needs and risks of adults requiring support. Developing, implementing, and reviewing care plans in partnership with service users and their families. Providing ongoing support and guidance to individuals, ensuring they receive appropriate services. Working closely with healthcare professionals, community organizations, and other stakeholders to facilitate holistic care. Maintaining accurate and timely records in compliance with local and national policies.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW). Registered with Social Work England. Strong knowledge of relevant legislation and best practices in adult social care. Excellent communication and interpersonal skills, with a strong ability to build rapport.
