Key responsibilities include:
Conducting comprehensive assessments of adult clients to identify their needs and strengths. Developing and implementing care plans in collaboration with clients, families, and other professionals. Providing information and guidance on available services and resources. Monitoring and reviewing client progress, adjusting care plans as necessary. Maintaining accurate and up-to-date case records in compliance with relevant legislation and policies. Collaborating with multidisciplinary teams to ensure holistic support for clients.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social work, particularly within a locality team setting. Strong assessment and analytical skills, with the ability to make sound judgments. Excellent communication and interpersonal skills, fostering positive relationships with clients and colleagues.
