Are you a passionate and dedicated social worker looking to make a difference in the lives of adults in Cardiff? We are seeking a qualified Adult's Social Worker to join our Locality Team. In this role, you will provide essential support and guidance to adults facing various challenges, ensuring they receive the care and services they need to thrive in their communities.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals' needs and circumstances. Developing and implementing tailored support plans in collaboration with service users and their families. Working closely with multidisciplinary teams to ensure holistic care delivery. Advocating for service users' rights and ensuring they are informed about their options. Maintaining accurate records and documentation in compliance with regulatory standards.To be considered for this role, you must possess the following qualifications and skills:
A recognized social work qualification (Degree in Social Work or equivalent). Current registration with Social Care Wales. Demonstrated experience in adult social work, particularly in community settings. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
