Your key responsibilities will include:

Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and strengths. Developing and implementing person-centered care plans in collaboration with service users, families, and other professionals. Providing ongoing support, advocacy, and guidance to individuals and their families. Monitoring and reviewing care plans to ensure the effectiveness of interventions and services. Collaborating with a multidisciplinary team and external agencies to facilitate holistic support.

To excel in this role, you will need to possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Proven experience working with adults with learning disabilities. Strong communication and interpersonal skills. Ability to work effectively within a team and independently. Knowledge of relevant legislation and best practices in social work.
