As a Social Worker in our LD Team, you will be responsible for:
Conducting comprehensive assessments to identify the needs of adults with learning disabilities. Developing and implementing tailored care plans in collaboration with individuals, families, and other professionals. Providing ongoing support, guidance, and advocacy to clients, ensuring their voices are heard. Facilitating access to appropriate services and resources, including health, education, and employment opportunities. Maintaining accurate and up-to-date case records in compliance with legislative and organizational standards.To be successful in this role, you will need to possess the following qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Proven experience in adult social work, specifically with learning disabilities. Strong assessment and analytical skills, with a commitment to person-centered practice.
