As a member of our team, your key responsibilities will include:
Conducting comprehensive assessments to identify the needs and preferences of individuals with learning disabilities. Developing and implementing tailored care plans in partnership with service users and their families. Providing ongoing support and guidance to clients, ensuring access to appropriate services and resources. Collaborating with healthcare professionals, community organizations, and other stakeholders to promote holistic care. Maintaining accurate records and documentation in compliance with statutory requirements and organizational policies.To be successful in this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work) and current registration with Social Work England. Experience working with adults who have learning disabilities, demonstrating a strong understanding of their unique challenges. Excellent communication and interpersonal skills, with the ability to build trusting relationships with clients and their families. Strong analytical and problem-solving skills, with a commitment to person-centered approaches.
