We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work - Learning Disabilities (LD) Team in Darlington. This role is crucial in supporting adults with learning disabilities, ensuring they receive the care and services necessary for a fulfilling life. As a vital member of our team, you will work collaboratively with individuals, their families, and other professionals to promote independence and well-being.

Your key responsibilities will include:

Conducting assessments to identify the needs and strengths of individuals with learning disabilities. Developing and implementing person-centered support plans that promote independence and enhance quality of life. Providing ongoing support and guidance to individuals and their families, ensuring access to appropriate resources and services. Collaborating with multidisciplinary teams, including healthcare professionals and community organizations, to coordinate care and support. Maintaining accurate records and documentation in compliance with legal and organizational standards.

To be considered for this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working with adults with learning disabilities in a social work capacity. Strong communication and interpersonal skills, with the ability to build rapport with clients and their families. Sound knowledge of relevant legislation and best practices in social work.
