We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work - Learning Disabilities Team in Cambridgeshire. In this rewarding role, you will play a vital part in supporting adults with learning disabilities, helping them lead fulfilling lives and achieve their personal goals. Your expertise will contribute to the development and implementation of tailored care plans, ensuring that individuals receive the support they need to thrive in their communities.
Key responsibilities include:
Conducting assessments to identify the needs and strengths of individuals with learning disabilities. Developing, implementing, and reviewing care plans in collaboration with service users and their families. Providing ongoing support and guidance, advocating for the rights and preferences of individuals. Collaborating with multidisciplinary teams and external agencies to coordinate services and resources. Maintaining accurate and up-to-date case records in compliance with regulatory standards.To be considered for this role, you must possess the following qualifications and skills:
A relevant degree in Social Work and current registration with Social Work England. Experience working with adults with learning disabilities in a social work capacity. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals. Proficient in assessment and care planning processes. Knowledge of relevant legislation and best practices in social work.Additional requirements include a valid driving license and access to a vehicle for work purposes.
