We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in Shropshire. This is an exciting opportunity for an individual looking to make a meaningful impact in the lives of adults facing complex health and social challenges. You will play a crucial role in supporting individuals through their hospital journey, ensuring they receive the appropriate care and resources necessary for a successful transition back to their community.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals’ social care needs and risks. Developing and implementing personalized care plans in collaboration with healthcare professionals, patients, and their families. Providing ongoing support and advocacy for patients, ensuring their voices are heard in care decisions. Coordinating discharge planning and connecting patients with community services and resources. Maintaining accurate and up-to-date records in accordance with legal and organizational standards.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience working within adult social care, preferably in a hospital setting. Strong assessment and analytical skills, with the ability to work under pressure.
