We are seeking a dedicated and experienced Qualified Social Worker to join our Adult's Social Work Hospital Team in Nottinghamshire. This is an exciting opportunity for a passionate professional to make a meaningful impact on the lives of adults in need of support within a hospital setting. The ideal candidate will demonstrate a commitment to safeguarding and promoting the welfare of vulnerable adults while working collaboratively with health and social care professionals.

Key responsibilities and duties include:

Conducting assessments of adults' needs and risks, ensuring compliance with relevant legislation and best practice. Developing and implementing care plans in partnership with individuals, families, and other stakeholders. Providing direct support and interventions to individuals and their families, facilitating access to necessary services. Collaborating with medical staff, community services, and external agencies to ensure holistic care delivery. Maintaining accurate records and documentation, adhering to data protection regulations.

Essential qualifications and skills required for this position include:

A recognized social work qualification (e.g., Degree in Social Work, Diploma in Social Work). Registration with Social Work England. Proven experience in adult social work, particularly within a hospital or healthcare setting. Strong assessment, analytical, and decision-making skills. Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals.
