This role is crucial in providing high-quality support and care to adults facing complex needs, ensuring they receive the appropriate services and interventions during their hospital stay and upon discharge.
As a key member of our Hospital Team, you will be responsible for:
Conducting assessments of individuals' needs and risks, ensuring a person-centered approach. Collaborating with healthcare professionals, families, and community resources to develop and implement care plans. Providing emotional and practical support to clients and their families, helping them navigate the healthcare system. Facilitating safe and timely discharges from hospital, ensuring continuity of care. Maintaining accurate and timely documentation in accordance with legal and organizational standards.To be successful in this role, you will need to possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social work, particularly within a hospital or health care setting. Strong assessment and analytical skills, with the ability to manage complex cases.
