We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's Social Work - Hospital Team in Hull. This is an exciting opportunity to make a meaningful impact on the lives of adults in need of support within a hospital setting. As a key member of our multidisciplinary team, you will work collaboratively to assess, plan, and implement effective care strategies for patients transitioning from hospital to home or other care settings.

Your key responsibilities will include:

Conducting comprehensive assessments of patients’ social care needs and developing care plans in collaboration with healthcare professionals. Providing support and guidance to patients and their families, ensuring they understand available resources and services. Working closely with community services to facilitate smooth transitions and continuity of care for discharged patients. Maintaining accurate and up-to-date case records and reports in compliance with organizational policies and procedures. Participating in team meetings and contributing to the development of best practices within the hospital environment.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Demonstrable experience in adult social work, preferably within a hospital or healthcare setting. Strong communication and interpersonal skills, with the ability to build rapport with patients and families. Excellent organizational skills and the ability to manage a varied caseload effectively.
