Join our dedicated Hospital Discharge Team in Somerset as an Adult's Social Worker, where you will play a crucial role in supporting individuals as they transition from hospital to home. In this dynamic position, you will work collaboratively with healthcare professionals, patients, and their families to ensure safe and effective discharges, providing the necessary support and resources to facilitate a smooth recovery process.
Your key responsibilities will include:
Conducting comprehensive assessments of patients’ social and emotional needs upon discharge. Developing and implementing personalized discharge plans in collaboration with multidisciplinary teams. Providing information and support to patients and their families regarding available community resources and services. Monitoring and reviewing patients' progress post-discharge to ensure ongoing support and adjustments to care plans as necessary. Maintaining accurate and up-to-date documentation in compliance with regulatory standards.To be successful in this role, you will need:
A recognized social work qualification (Degree in Social Work or equivalent). Registration with Social Work England. Experience in adult social work, particularly in hospital discharge settings. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
