An experienced and committed Adult Social Worker is required to join a Disability Team supporting adults with physical disabilities, sensory impairments, learning disabilities and complex support needs. You will work in a person-centred way to promote independence, wellbeing, choice and control, helping individuals achieve positive outcomes in their daily lives. This is an opportunity to contribute to a responsive service while working with adults, families, carers and partner agencies across a diverse community.
Your responsibilities will include completing Care Act assessments, support planning, financial assessments and statutory reviews; identifying strengths, risks and eligible needs; and arranging appropriate care and support packages. You will manage a varied caseload, undertake safeguarding enquiries, contribute to hospital discharge planning and support transitions where required. The role will also involve applying the Mental Capacity Act and relevant legislation, completing best-interest decisions, making effective use of direct payments and personal budgets, and ensuring that services promote least restrictive and proportionate outcomes.
Applicants must hold a recognised Social Work qualification and current registration with Social Work England, alongside relevant post-qualifying experience in adult social care or a related setting. You should have a sound understanding of the Care Act, safeguarding procedures, mental capacity, disability services and strengths-based practice. Excellent communication, assessment, recording and organisational skills are essential, as is the ability to manage competing priorities and work confidently with professionals, providers, families and people who use services. A proactive approach, professional integrity and commitment to equality, inclusion and positive risk-taking are also required. The ability to travel within the local area and undertake visits in community settings is essential.
