We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's Social Work Community Team in Herefordshire. This role is vital in providing support and intervention for adults in need, ensuring they receive the highest quality of care and assistance. As a key member of our team, you will work collaboratively with individuals, families, and other professionals to promote wellbeing and safeguard vulnerable adults within the community.

Your key responsibilities will include:

Conducting comprehensive assessments of individuals’ needs and risks. Developing and implementing tailored care plans in collaboration with service users and their families. Providing ongoing support and intervention, ensuring individuals have access to necessary resources and services. Maintaining accurate records and documentation in accordance with regulatory requirements. Participating in multi-disciplinary meetings and contributing to safeguarding initiatives.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., BA in Social Work, MA in Social Work). Current Social Work England registration. Proven experience in adult social work, particularly within a community setting. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals. Knowledge of relevant legislation and frameworks, including the Care Act and safeguarding protocols.
