We are currently seeking a dedicated and experienced Adult's Social Worker to join our Community Team in Cardiff. As a key member of our team, you will be responsible for providing high-quality social work services to vulnerable adults in the community, ensuring their safety and well-being.

Key responsibilities and duties include:

Conducting assessments and reviews to identify the needs of service users Developing care plans and support packages tailored to individual needs Working collaboratively with other professionals and agencies to provide holistic support Advocating for the rights and best interests of service users Participating in multidisciplinary meetings and case conferences

The ideal candidate will possess the following essential qualifications and skills:

Qualified Social Worker with a valid registration with Social Care Wales Demonstrated experience in adult social work, preferably within a community setting Strong assessment and decision-making skills Excellent communication and interpersonal skills Ability to work effectively under pressure and manage a complex caseload
