We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - CHAT Team in Barking & Dagenham. This role is crucial in providing high-quality support and safeguarding services to adults in our community. You will work collaboratively within a multi-disciplinary team, ensuring that individuals receive the care and support they need to lead fulfilling lives.
Your key responsibilities will include:
Conducting comprehensive assessments of adults' needs and risks to inform care planning. Developing and implementing person-centered support plans in collaboration with service users and their families. Providing ongoing support, advocacy, and guidance to adults facing various challenges. Maintaining accurate and up-to-date case records in compliance with local policies and statutory requirements. Working closely with external agencies and stakeholders to ensure a holistic approach to service delivery.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., CQSW, DipSW, or equivalent). Registered with Social Work England. Demonstrable experience in adult social work, particularly in community settings. Strong assessment and analytical skills, with the ability to make informed decisions.
