As a member of our team, you will be responsible for assessing the needs of individuals, providing tailored support plans, and ensuring the highest quality of care is delivered. Your key responsibilities will include:
Conducting comprehensive assessments of service users’ needs and risks. Developing and implementing care plans in collaboration with service users and their families. Providing ongoing support and monitoring of care plans to ensure effectiveness and compliance. Working closely with multidisciplinary teams, including health professionals and community organizations. Maintaining accurate documentation and case records in accordance with organizational policies.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Proven experience in adult social work, particularly in community care settings. Strong assessment and analytical skills with a person-centered approach.
