Join our dedicated Assessment Team in Solihull as a Qualified Social Worker, where you will play a vital role in supporting adults in need of social care services. In this position, you will conduct comprehensive assessments, develop care plans, and ensure that individuals receive the appropriate support tailored to their unique circumstances. Your expertise will help empower individuals to achieve their goals, promoting independence and well-being within the community.
Your key responsibilities will include:
Conducting thorough assessments of individuals’ needs and risks. Developing and reviewing care plans in collaboration with service users and their families. Working closely with multidisciplinary teams to ensure a holistic approach to care. Providing guidance and support to less experienced team members. Maintaining accurate and up-to-date case records in compliance with legislation.To be successful in this role, you will need to have the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience in adult social work, particularly in assessment and care planning. Strong communication and interpersonal skills to engage effectively with service users and stakeholders.
