Join our dedicated Assessment Team in Lancashire as a Qualified Social Worker specializing in Adult Services. We are seeking a passionate and experienced professional to support vulnerable adults through comprehensive assessments, ensuring they receive the care and support they need. Key responsibilities include:
Conducting thorough assessments of adults in need of support, identifying their needs and strengths. Developing, implementing, and reviewing care plans in collaboration with service users and their families. Providing expert advice and support to colleagues and stakeholders regarding adult safeguarding issues. Maintaining accurate and up-to-date case records in accordance with legal and organizational standards. Participating in multi-disciplinary meetings and contributing to the development of service improvements.To be successful in this role, candidates must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work, DipSW). Registration with Social Work England. Proven experience in adult social work, particularly in assessment and care planning. Strong communication and interpersonal skills, with the ability to build rapport with service users.
