We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's Social Work – ANT Team in Worcestershire. As a key member of our team, you will be responsible for assessing and supporting individuals with complex needs, ensuring they receive the highest standard of care and support tailored to their individual circumstances.
Your key responsibilities will include:
Conducting thorough assessments of clients’ needs and risks, ensuring person-centered approaches are prioritized. Developing and implementing care plans in collaboration with clients, families, and other professionals. Providing ongoing support and guidance, facilitating access to necessary resources and services. Maintaining accurate and up-to-date case records in compliance with local and national standards. Working collaboratively with multidisciplinary teams to promote integrated service delivery.To be successful in this role, you will need to possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Demonstrated experience in adult social work, preferably within an assessment and safeguarding context. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
