Key responsibilities include:

Conducting thorough assessments of clients' needs and circumstances. Developing and implementing personalized care plans in collaboration with clients and their families. Providing ongoing support and advocacy to help clients access necessary services and resources. Monitoring progress and adjusting care plans as needed to ensure optimal outcomes. Maintaining accurate and up-to-date documentation in compliance with regulatory standards.

To be successful in this role, you will need the following essential qualifications and skills:

A recognized social work qualification (e.g., BA/MA in Social Work). Registration with the relevant social work regulatory body. Experience working with adults in a social work setting. Strong communication and interpersonal skills, with the ability to build rapport with clients. Excellent organizational skills and the ability to manage a varied caseload.
