Your key responsibilities will include:
Conducting comprehensive assessments of older adults’ needs and preferences. Developing and implementing personalized care plans in collaboration with clients and their families. Working closely with healthcare professionals, community organizations, and other stakeholders to ensure integrated service delivery. Monitoring and reviewing care plans to ensure effectiveness and adapt to changing needs. Providing guidance and support to families and caregivers, empowering them in their roles.To be successful in this role, you will need to hold a relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent) and be registered with Social Work England. Additionally, you should possess:
Strong knowledge of legislation and best practices related to adult social care. Excellent communication and interpersonal skills, with the ability to build rapport with clients. Experience in working with older adults and understanding their unique challenges. Ability to work independently and as part of a multidisciplinary team.
