We are seeking a dedicated and compassionate Adult's Social Worker (SSW) to join our Mental Health Team in Wakefield. Your key responsibilities will include:
Conducting comprehensive assessments of individuals’ mental health needs. Developing, implementing, and reviewing personalized care plans in partnership with service users. Providing ongoing support and intervention to facilitate recovery and independence. Collaborating with external agencies and stakeholders to coordinate services effectively. Maintaining accurate and up-to-date case records in compliance with regulatory standards.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., CQSW, CSS, or equivalent). Registration with Social Work England or equivalent regulatory body. Experience working within mental health settings, particularly with adults. Strong assessment and analytical skills, with the ability to manage complex cases.
