We are seeking a dedicated and compassionate Adult's Social Support Worker (SSW) to join our Care Home Team in Manchester. Your key responsibilities will include:

Assessing the needs of residents and developing personalized care plans. Providing emotional and practical support to residents and their families. Facilitating daily activities and encouraging social interaction among residents. Monitoring and reporting changes in residents' health and behavior. Collaborating with healthcare professionals to ensure comprehensive care.

To be successful in this role, you must possess the following essential qualifications and skills:

A relevant degree in Social Work or equivalent qualification. Registered with Social Work England (SWE). Experience in adult social care or a related field. Strong communication and interpersonal skills. Empathy, patience, and a genuine passion for helping others.
