We are seeking a dedicated and compassionate Adult's Social Support Worker (SSW) to join our AEST Team in Manchester. This role is pivotal in providing high-quality support to adults with varying needs, ensuring they receive the assistance necessary to lead fulfilling lives. If you are passionate about making a difference in the community and possess the skills to support individuals in navigating their daily challenges, we want to hear from you.

As an Adult's SSW, your key responsibilities will include:

Conducting assessments to identify the needs and preferences of service users. Developing and implementing personalized care plans in collaboration with service users and their families. Providing emotional and practical support to individuals, helping them achieve their goals. Coordinating with other health and social care professionals to ensure comprehensive support. Monitoring and reviewing progress, adjusting care plans as necessary. Maintaining accurate records and documentation in compliance with local policies and regulations.

The ideal candidate will possess essential qualifications and skills, including:

A relevant degree in Social Work or equivalent qualification. Registration with the appropriate professional body (e.g., Social Work England). Experience working with adults in a social care or support setting. Strong communication and interpersonal skills, with the ability to build rapport with service users. Empathy, patience, and a commitment to promoting independence and well-being.
