Join our dedicated team at Future Options Supported Living in Sheffield as an Adults Social Worker, where you will play a vital role in enhancing the quality of life for individuals in our care. As a key member of our multidisciplinary team, you will be responsible for assessing the needs of adults with varying support requirements and developing tailored care plans that promote independence and well-being.
Your key responsibilities will include:
Conducting comprehensive assessments to identify needs and risks. Developing, implementing, and reviewing care plans in collaboration with service users and their families. Providing support and guidance to service users, ensuring their voices are heard in the decision-making process. Working closely with other professionals, agencies, and community resources to provide holistic support. Maintaining accurate and up-to-date case records in accordance with regulatory standards.To be successful in this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., CQSW, DipSW, or equivalent). Registration with Social Work England. Experience in adult social work, particularly in supported living or community settings. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
