We are seeking an experienced and compassionate Adult Occupational Therapist to join a community-based service supporting adults to remain independent, safe and engaged in everyday life. You will work with people who may have physical disabilities, long-term conditions, neurological needs, mental health conditions or age-related challenges. The role involves completing comprehensive assessments, identifying individual goals and delivering practical, person-centred interventions that promote wellbeing and maximise independence.
Your responsibilities will include assessing functional ability in the home and community, recommending appropriate equipment and adaptations, advising on manual handling and falls prevention, and supporting hospital discharge and admission avoidance. You will work collaboratively with service users, families, carers and professionals from health and social care to develop and review effective care plans. You will maintain accurate, timely records, prepare reports, manage a varied caseload and contribute to safeguarding, risk management and service improvement. You may also provide guidance to support workers, students and other colleagues, sharing occupational therapy expertise across the wider team.
You will need a degree or equivalent qualification in Occupational Therapy and current professional registration. Experience of working with adults in a community, social care, rehabilitation or hospital setting is desirable, together with sound knowledge of assessments, equipment provision, moving and handling, housing adaptations and relevant legislation. Excellent communication, organisation and decision-making skills are essential, as is the ability to work autonomously while contributing positively to a multidisciplinary team. A flexible, respectful and solution-focused approach is required, along with a commitment to equality, dignity, confidentiality and person-centred practice. The ability to travel between community locations and undertake visits in service users’ homes is also essential.
