We are seeking a dedicated and compassionate Adult Social Worker specializing in Learning Disabilities (LD) and young people transitioning to adulthood to join our dynamic team in Derby. In this role, you will play a crucial part in supporting individuals with learning disabilities as they navigate the complexities of adulthood, ensuring they receive the care and resources they need to thrive. Your expertise will help empower young people to achieve their goals and enhance their quality of life.
Key responsibilities include:
Conducting comprehensive assessments of individuals’ needs and developing tailored support plans. Collaborating with multidisciplinary teams to coordinate service delivery and ensure holistic support. Providing advocacy and guidance to individuals and their families, helping them understand available resources. Monitoring and reviewing progress to adapt support plans as necessary, ensuring positive outcomes. Maintaining accurate and up-to-date records in compliance with organizational policies and statutory requirements.The ideal candidate will possess the following qualifications and skills:
A relevant Social Work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England. Experience working with adults with learning disabilities and knowledge of transition processes. Strong interpersonal skills with the ability to build trusting relationships.
