We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team in Wolverhampton, specializing in working with individuals with Learning Disabilities (LD) and Physical Disabilities (PD). As a key member of our social work team, you will play a crucial role in supporting and advocating for vulnerable individuals in the community.

Key responsibilities and duties:

Conducting comprehensive assessments of service users' needs and developing individual care plans Providing direct support and intervention to individuals with LD and PD, promoting independence and well-being Liaising with other professionals and agencies to coordinate holistic care and support services Participating in safeguarding procedures and ensuring the protection of vulnerable adults Keeping accurate and up-to-date records of assessments, interventions, and outcomes

Essential qualifications and skills:

Qualified Social Worker with a recognized social work qualification (e.g., BA in Social Work) Registered with Social Work England and eligible to work in the UK Demonstrated experience in working with individuals with LD and PD in a social work or similar setting Excellent communication and interpersonal skills, with the ability to build rapport with service users and their families Strong assessment and case management skills, with a person-centered approach to care
