Your key responsibilities will include:

Conducting comprehensive assessments of adults’ needs and risks. Developing, implementing, and reviewing person-centered care plans. Collaborating with multi-disciplinary teams and external agencies to ensure holistic support. Providing emotional and practical support to individuals and their families. Maintaining accurate and up-to-date case records in compliance with legal and organizational standards.

To be successful in this role, you will need the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Demonstrable experience in adult social work, particularly within a community or neighbourhood setting. Strong assessment and analytical skills, with the ability to make informed decisions. Excellent communication and interpersonal skills, with a compassionate approach to service users.
