Join our dedicated Locality Team in North Lincolnshire as a Qualified Social Worker, where you will play a vital role in enhancing the lives of adults within our community. As a key member of our team, you will be responsible for conducting assessments, developing care plans, and providing ongoing support to individuals and families. Your expertise will help ensure that vulnerable adults receive the necessary services and interventions to promote their well-being and independence.

Your key responsibilities will include:

Conducting comprehensive assessments and reviews of adult service users. Developing and implementing person-centered care plans in collaboration with service users and their families. Providing direct support and guidance to individuals, promoting their rights and choices. Working collaboratively with other professionals, agencies, and stakeholders to ensure effective service delivery. Maintaining accurate and up-to-date case records and documentation in accordance with legal and organizational standards.

To be successful in this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Strong knowledge of relevant legislation, policies, and best practices in adult social care. Excellent communication and interpersonal skills, with the ability to build rapport with diverse clients. Proven experience in case management and working with adults who have complex needs.
