Your key responsibilities will include:

Conducting comprehensive assessments of patients' social care needs and discharge requirements. Collaborating with multidisciplinary teams to develop and implement effective discharge plans. Providing information and support to patients and their families to ensure understanding of discharge processes. Identifying and coordinating community resources and services to support patient reintegration. Maintaining accurate records and documentation in compliance with legal and organizational standards.

To be successful in this role, you will need the following qualifications and skills:

A recognized social work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England. Proven experience in a hospital setting, particularly in discharge planning. Strong communication and interpersonal skills to engage effectively with patients, families, and professionals. Excellent organizational skills and the ability to manage competing priorities.
