We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Hospital Discharge/Assessment Team in Manchester. Key responsibilities include:

Conducting comprehensive assessments of patients' needs and circumstances. Developing and implementing person-centered discharge plans in collaboration with multidisciplinary teams. Providing emotional support and guidance to patients and their families during the discharge process. Ensuring compliance with relevant legislation and best practice guidelines. Maintaining accurate case records and documentation in accordance with organizational policies.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Experience in adult social work, particularly in hospital discharge or assessment settings. Strong communication and interpersonal skills, with the ability to work effectively in a team. Knowledge of relevant legislation, policies, and procedures related to adult social care.
