We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team in Conwy. This position focuses on providing support and assistance to older people in hospital settings, making a real difference in their lives. The hourly rate for this role is £35.98.

Key responsibilities and duties:

Assess the needs of older people in hospital and develop care plans tailored to their individual requirements. Provide emotional support and advocacy for patients to ensure their well-being and rights are protected. Collaborate with healthcare professionals, families, and external agencies to coordinate services and resources for older people. Monitor and review care plans regularly to ensure they are effective and meeting the changing needs of patients.

Essential qualifications and skills:

Qualified Social Worker with relevant registration and experience in working with older people. Strong assessment and communication skills to engage effectively with patients and multidisciplinary teams. Knowledge of relevant legislation, policies, and procedures related to social work practice in healthcare settings. Ability to work independently, make sound decisions, and prioritize workload in a fast-paced environment.
